In pursuance of the orders of Lahore High Court, an Entrance Test shall be conducted for candidates, desirous of admission to medical / dental institutions of the Punjab, who have secured 60% or above marks in FSc (pre-medical) or equivalent examination, and who could not appear in the Entrance Test held on 20th July,2010. The test shall be held on Thursday, 14^th October, 2010, at 02:30 PM, at Lahore.

1. WHO CAN APPLY
   1. Candidates having domicile of any district of Punjab, Federal Capital Area (Islamabad), Azad Jammu & Kashmir, Gilgit-Baltistan, Overseas Pakistanis and Foreign Nationals can apply.
2. HOW TO APPLY
   1. Entrance Test Application Form, Admittance Cards (both copies) and Information Booklet are available on UHS website and can be downloaded free of cost.
   2. Duly filled Entrance Test Application Form and Admittance Cards (both copies) must be submitted by hand at University of Health Sciences (UHS) Lahore from 9^th to 11^th October, 2010(including Sunday) from 08:30 AM to 02:30 PM, with one set of attested copies of following documents:
      1. Matric (SSC or equivalent) certificate
      2. FSc (pre-medical) or equivalent certificate.
      3. Domicile certificate of the candidate.
      4. Three passport size coloured photographs. (One to be pasted on the application form and to be attested on the front and other two, attested on the back, to be attached with the application form).
      5. Two self addressed envelops with Rs.30/- postage stamps each.
3. IMPORTANT
   1. Candidates with less than 60% marks in FSc or equivalent examination are NOT eligible to apply.
   2. Applications received by post shall NOT be entertained. Online submission is also NOT acceptable.
   3. Applications received after 02:30 PM on 11th October, 2010 shall NOT be entertained.
   4. Complaint, if any, against the result, may be lodged within 07 days of the declaration of official result. No complaint shall be entertained thereafter.
   5. All those candidates who have already appeared in the Entrance Test held on 20th July, 2010, need not apply.
